Gadolinium Contrast Media

The secret behind the mode of action of MRI contrast media lies in their ability to interfere with the electromagnetic behavior of tissues, leading to improved contrast on an MR image. Most MRI contrast agents are based on the rare earth element gadolinium, adding paramagnetic properties to the compound. Gadolinium causes the atomic nuclei to relax more quickly and thus to transmit stronger signals, leading to an improved contrast.

Iodinated Contrast Media

The tolerability and performance of iodinated contrast media for use in X-ray and CT procedures are strongly influenced by three physicochemical properties: osmolality, viscosity, and iodine concentration. These are interrelated and depend on the structure and size of the iodine-binding molecule. Low-osmolar contrast agents have a better safety profile than high-osmolar contrast agents.

Contrast-induced Nephropathy

Recently, the renal tolerance of iodinated X-ray contrast media has been disputed in patients with pre-existing renal insufficiency, especially if they suffer from diabetes. As reviews and guidelines consistently agree, hydration is the cornerstone for the prophylaxis of contrast-induced nephropathy (CIN).
